Universal Free School Meal Info Form for Pupils in Years 4-6 in September 2023 The local authority are planning to roll-out free school meals to Years 3-6 from September 2023. This is an updated form - please complete. The responses will go directly to the catering team. The local authority needs to ensure that eligible families are still claiming their FSM entitlement to safeguard the additional grants that schools receive. The form also acts as the gateway for parents to access additional grants for clothing. Special diet information is also collected on these forms to ensure that we are aware of any dietary requirements pupils have to ensure they are provided with a safe and nutritious meal.
